Marriage Contract for female married applicants (3pcs Photocopy)

Passport size ID picture (4pcs)

1x1 ID Picture (3pcs)

Diploma/TOR/Form 138/ALS Certificate

Medical Certificate

Barangay Clearance/Police Clearance/NBI Clearance

Tentative Schedule: April 2024 (Monday to Friday 1pm to 9pm)
